NOTICE OF CALL OF ELECTION FOR WHITE

MOUNTAIN COMMUNITIES SPECIAL

HEALTH CARE DISTRICT

Notice is hereby given that White Mountain Communities

Special Health Care District (the “District”)

will hold and conduct an election on Tuesday,

November 6, 2018 for the following purposes:

• to obtain approval of a majority of the qualified

electors voting in the election to the continued

imposition on an annual basis of a secondary

property tax on the taxable property within the

boundaries of the District, at the rate of $.66 per

$100 of assessed valuation, for the purpose of

paying the costs of maintaining and operating the

District’s facilities and for professional and other

services to the District; provided, the aggregate

amount to be levied in any fiscal year shall not

exceed the maximum levy permitted by law. The

continued imposition of such tax to be subject to

the approval of a majority of the qualified electors

of the District at least every ten (10)years;

• to obtain the continuing approval of a majority

of the qualified electors voting in the election to

use a portion of the funds received by the District,

from a secondary property tax the District is

authorized to cause to be levied, to maintain and

operate an ambulance service or pay the costs

of an ambulance service contract; the continued

authorization for such use of funds to be subject

to the approval of the qualified electors of the District

at least every five (5) years; and

• to elect five (5) persons, one from each directorship

district in the District, to serve as directors

of the District, for four (4) year terms.

Individuals who wish to become candidates to be

a director of the District must have the qualifications

prescribed by Ariz. Rev. Stat. § 48-5502 and

must file nomination papers and petitions no later

than 5:00 o’clock p.m. on August 8, 2018 with the

Clerk of the Apache County Board of Supervisors,

or as directed with the Elections Director of

Apache County, Arizona.

Persons who reside within the District and who

possess the qualifications required of electors

under the general election laws of the State of

Arizona shall be eligible to vote in the election.

The last date to register to vote in the election is

October 8, 2018.
